Please indicate which Learning Management System (LMS) your institution employs.

If you, personally, have administrative privileges in your LMS, please check the box. If not, leave it blank.

Provide the first and last name of the person at your institution who is responsible for administration of your LMS.

Some institutions provide multiple, different URLs for users to login. For example, instructors may use one URL, while students use a different URL. Or you may have a long URL provided by your LMS, as well as a shorter "vanity" URL that was purchased by your instituion, and they both point to the same place. If this is the case for your school, please check this box so we can set up your course manager to prevent conflicts.
